Welcome to the studio booth registration form for the Saskatoon Tattoo Expo.  We have an extensive list of amazing tattoo artists / studios who attend our shows each year and we want to be able to accommodate everyone.  If you would like to be considered please register via the form below.  Everyone has to register regardless of past attendance.  If you have any questions please send to booths@saskatoontattooexpo.ca.  Approved artists will be added to the waiting list.

To cause as little confusions as possible, you must follow through this registration process carefully. You will register below which will notify our team, and once we have reviewed and approved, you will be emailed an invoice and payment link to the billing email provided during the registration process. We must receive payment for your booth before you will be listed on the website.  The closer it is to the show that you pay for your booth, the longer it will take for you to appear on the website as time becomes more limited for us.

Please follow and read the instructions carefully, having names, phone, email and social media handles readying, including example artwork for submission.

Please allow yourself appropriate time to fill out this registration as page 2 includes health paperwork required to attend the show.

Hand Washing

The hand wash sink must be equipped with:
✓ hot and cold water under pressure;
✓ liquid soap in a dispenser;
✓ disposable paper towels in a dispenser;
✓ lined garbage container; and
✓ an enclosed waste water collection tank

Equipment Used for Invasive Personal Services

Please list all equipment that will be used during services and indicate if equipment will be
disposable or reusable. Note: Please attach a separate sheet of paper if more space is
required

Note:

Packaged sterilized instruments must be protected from contamination during transit and
while in storage at the temporary location. Used instruments are placed in detergent water
solution in a secure, leak proof, puncture resistant container before being transported to the base
of operation for reprocessing.

Booth Layout

You are required to submit a diagram of your personal services booth. The booth diagram
should depict the location of all hand wash stations, equipment, tables, storage areas and
garbage bins. Please clearly label each item in the booth. A blank booth diagram for you to
use can be found on page 8 of this document.

Pre-Opening Checklist

The operator of a personal services booth should ensure this checklist is complete prior to
opening. Ensure all applicable items are available at a minimum of one hour prior to the
start ofthe event, and again every day prior to opening (if it is a multi-day event). Failure to
have the following conditions met may result in delays and you may not be permitted to
operate.

The information I have provided is complete and accurate to the best of my knowledge. I
agree to comply with the provisions of the Temporary Personal Services at Events
Information Sheet: Invasive Services, Personal Service Facility Best
Management Practices, and the Health Hazard Regulations, any other instructions I receive
from the Public Health Inspector. I understand that failure to meet the requirements will
result in not being allowed to operate. I understand that I may not receive approval if the
application is incomplete or not submitted at least two weeks prior to the event. I
understand that I cannot operate without approval from the local health authority.
